Services that health insurance covers:

We check your benefits with your insurance carriers and notify you regarding what your insurance will cover and we get the needed doctor's orders.  Insurance pays for services your doctor orders such as:

• Nurse Visits to help you adjust to a new condition, monitor unstable health issues,  do specialized wound care, manage medications or obtain laboratory samples.
• Therapies in the home. Physical therapy and Occupational therapy assure safety in the home and help people return to their maximum functioning.
• Home Health Aide visits to help with a bath, exercises, etc.

Special features offered to Sunrise Health clients:

• 24 hour Nurse On-Call
• Focused Monitoring
• For 30 days following discharge from a hospital or nursing home  more intense monitoring and visit frequency are provided to address any unstable conditions and prevent avoidable problems.
• Point of Care testing
• Immediate readings and dose adjusting for clients on blood thinners
• Home Telehealth Monitoring
• Disease management and early intervention to prevent crises are accomplished through home daily monitoring. When appropriate, a unit is installed in the home and readings of blood pressure, pulse, weight, blood oxygen, etc. are transmitted to the health professionals for evaluation. Sunrise Health nurses and the patient's physician then tailor the cares to meet the patient's health needs.
• Infusion Therapy
• No need to stay in a facility to receive IV antibiotics. Specially trained nurses work with your medical team to provide specialized care in the home.

Sometimes all a parent or loved one needs to remain independent at home is a little extra support. We are flexible and have the resources to help with personal care, shopping, and household management. We can also accompany and arrange transportation services to help loved ones get out and about to community and family events.

Other services include Personal care, Housekeeping, Chronic disease management.
